These Terms of Service (“Terms”) govern your use of the website, protocol assessment, forms, and text-message program of The Charleston Atelier, the physician-led practice of Kendall Phelps-Polirer, MD (“we,” “us,” or “our”). By using our website or submitting a form, you agree to these Terms.
Not medical advice
The information on this website and in our protocol assessment is educational and general in nature. It does not constitute medical advice, diagnosis, or treatment, and does not establish a physician–patient relationship. Any treatment recommendation requires a physician consultation, review of your medical history, examination as appropriate, and appropriate informed consent. If you have a medical emergency, call 911.
Eligibility and use of the site
You must be 18 years or older to use this website and to submit your information. You agree to provide accurate information and to use the site only for lawful purposes. You may not attempt to disrupt the site or access it in an unauthorized manner.
Appointments and assessment
Scheduling a consultation or completing the protocol assessment does not guarantee that any particular treatment, medication, or protocol is appropriate or available for you. Final recommendations are made by the physician based on clinical evaluation.
Text-message (SMS) program
The Charleston Atelier offers optional text messages about appointment scheduling, appointment reminders, and follow-up on your inquiry. With your separate marketing consent, we may send special offers, discounts, and practice updates. Each consent choice is optional and is not a condition of purchase. Message frequency varies; message and data rates may apply.
